Request Form
Please be patient as I process your request for classes or camp. A request form is not a guarantee you have a spot held for you. You will be notified in an email as to whether a spot is available or you have been added to a waitlist.
Please note that Chickadee classes with less than 3 students, and Sparrow classes with less than 4 students are subject to cancellation. Once the request form is submitted you will be emailed the Medical Form, Pick-Up Authorization Form and Liability Waiver to submit with payment to confirm your child’s enrollment.
A NON-REFUNDABLE Deposit ($200 for camp, $350 for school year classes) is included in the cost of tuition.
Proof of all the standard (pre-covid) immunizations is required for participation in school year classes.
